Employee Information Reminder

• Before you begin an Agency application, please be prepared with the names and email addresses of all employees you wish to include.
• You will be able to enter information for up to three employees directly within the form. If your agency has more than three employees, you’ll have the option to upload an Excel or CSV file containing the full list of names and email addresses during the application process.
